What Dermatologists Want Black Patients to Know About Follicular Eczema

Follicular eczema often appears as small, rough bumps that mimic goosebumps or razor burn. On Black and brown skin, these bumps can be more pronounced and may leave behind dark marks. Dermatologists stress that this condition is not the same as standard body acne or ingrown hairs. The key distinction lies in where the inflammation occurs, specifically around the hair follicles.

Exfoliating the bumps away will not work. In fact, aggressive scrubbing or using harsh acids can worsen the irritation and lead to more visible hyperpigmentation. The root cause is not a buildup of dead skin cells on the surface. Instead, it is an inflammatory response within the follicle that requires a gentler, targeted approach.

Many patients mistake follicular eczema for keratosis pilaris, commonly known as chicken skin. Both conditions cause bumps, but they have different triggers and treatment paths. Keratosis pilaris involves a buildup of keratin blocking the follicle, while follicular eczema is linked to an overactive immune response. A dermatologist can differentiate between the two through a physical exam and sometimes a biopsy.

Moisturizing is a critical first step, but not all moisturizers are equal for this condition. Thick creams and ointments that contain ceramides or shea butter help repair the skin barrier. Lotion with a high water content can evaporate quickly and leave the skin drier. For Black patients, maintaining hydration is especially important because drier skin can amplify the appearance of bumps and dark spots.

Topical steroids are often prescribed to reduce inflammation at the follicle level. Over-the-counter hydrocortisone may help for mild cases, but stronger prescription options are reserved for persistent flares. When used under a dermatologist’s guidance, these treatments can clear bumps without thinning the skin. Long-term use requires monitoring, especially on areas like the arms and legs.

Heat and friction are common triggers for follicular eczema flares. Tight clothing, heavy sweating, and even certain workout fabrics can irritate the hair follicles. Dermatologists recommend loose, breathable clothing and showering promptly after exercise. For Black patients, hair care routines that involve tight braids or heat styling can also contribute to scalp or neck flare-ups.

Dark spots, or post-inflammatory hyperpigmentation, often outlast the actual eczema bumps. Treating the inflammation early is the best way to prevent these marks from setting in. Once hyperpigmentation develops, options like azelaic acid or prescription lightening agents may be used, but only after the eczema is controlled. Sun protection is essential, as UV exposure darkens these spots further.

Seeking a dermatologist who understands textured skin is vital. Many general treatment guidelines are based on lighter skin tones and may miss the subtleties of follicular eczema on deeper complexions. A specialist can tailor a routine that respects the skin’s natural oil production and barrier needs. Patients should not hesitate to ask for a provider with experience in skin of color.

Managing follicular eczema is a long-term process, not a quick fix. Consistency with a gentle cleansing, moisturizing, and anti-inflammatory routine is more effective than any occasional intensive treatment. Lifestyle adjustments, such as avoiding known triggers and wearing softer fabrics, play an equally important role. With the right medical guidance, patients can achieve clearer skin without compromising their skin’s health.
